Payment Policy:
Payment each month will be due on the first. You must set up your own monthly autopay NO LATER than March 1st in order for your child to attend class on the first day. Please note, whatever date you complete the first payment is the date your monthly autopay will occur (i.e. 3/1, 4/1, 5/1, etc).
You agree to pay the amount agreed and not to cancel the transactions with your bank or credit card company. You are committing to the full session indicated at sign up. Program participation is not on a week to week basis, and as such you will be responsible for the full session of tuition. Roots & Wings Village is not responsible for any overdraft charges, over limit charges, or NSF fees charged by your bank or credit card company.
If fees are not paid in full and on time, you agree to pay a $25.00 late fee. If past due balances are not paid within 10 days, the child(ren) will not be allowed to attend the program until payment is received in full.
All tuition paid is nonrefundable. If your child is withdrawn from our program while a balance remains on their account, he/she cannot return until the balance has been paid in full.
Late Pickup Policy:
In the event of late pick-up, families will be charged a fee of $5 for every 15 minutes late. Pickup for the elementary and middle school enrichment is between 3-3:15pm. Please make every effort to be on time, as our small team needs adequate time to clean up our spaces at the end of the day. In the event of extenuating circumstances, please notify Anne (646)467-0376
Sick Policy:
Parent/caregiver will need to complete a daily assessment for any signs of illness. Parents/caregivers should keep the child at home if:
The child has a fever. Fever reducing medication may not be used when determining a child’s health. Children must be fever-free for 24 hours before returning to Roots and Wings Village.
The child is experiencing ANY of the recognizable symptoms of illness, including a fever, dry cough, headache, fatigue, nausea, vomiting, diarrhea, sore throat, or difficulty breathing.
The child is under a self-quarantine/self-monitoring guidance for any reason.
If a child develops ANY recognizable symptoms of illness (including a fever, dry cough, headache, fatigue, nausea, vomiting, diarrhea, sore throat, or difficulty breathing) while at Roots and Wings Village:
Staff will remove the child from the group and relocate them to an isolated area with a member of staff.
Staff will call the caregiver to pick the child up immediately.
Cancellation Policy
50% refunds will be provided for cancelled classes, unless class is rescheduled for an alternate date in which case refunds will not be made available. We do not provide refunds for individual absences due to illness or schedule conflicts.
Behavior Policy & Expectations:
We believe that all members of a group flourish when they know how they are expected to behave and when they are free to develop their play and learning without fear of being hurt or unfairly restricted by anyone else. We are committed to establishing a learning environment in which group members develop self-discipline and positive self-esteem as they take increasing responsibility for themselves and their actions, and as they consider the well being of others. We actively promote positive behavior, where children treat each other with care and respect and focus on activities that require sharing, negotiation and cooperation. We support all participants to develop their understanding of risk taking and dealing with challenges.
Our sessions enable participants to learn through what they encounter and through what they discover to actively learn the skills of enquiry, experiment, feedback, reflection, review and cooperative learning. We will use positive strategies for resolving conflict by helping children and others to find age/developmentally appropriate solutions.
Participants are encouraged to be responsible for their own actions and belongings, and are expected to treat themselves, others, and all classroom materials with respect.
Unacceptable behavior will be addressed in a calm and assertive manner. It will be made clear that it is the behavior that is unacceptable, not the person. Serious incidents or persistent occurrences of unacceptable behavior will be recorded on incident forms and could lead to exclusion from Roots & Wings Village programs.
Staff members will only use physical restraint, such as holding, to prevent physical injury to children or adults and/or serious damage to property. Details of such an event (what happened, what action was taken and by whom, and the names of witnesses) are recorded in our Incident Records. A parent/caregiver is informed on the same day and will be provided a copy of the incident report.
Behavior Policy & Expectations (continued):
In the event of ongoing unresolved behavioral issues, a conference meeting will be requested between the program director, the child(ren), and the parent/caregiver(s). We strive to find a mutually agreeable solution to the issue(s) together.
In the event the program director believes the child needs direct (1-1) support in order to maintain enrollment in the program, the parent will agree to provide a support person at their own expense (either by hiring an outside support professional or by shadowing their child themselves during the program’s operational hours).
Termination of Services:
In the event a family wishes to terminate childcare services prior to the end of a full session, the parent/guardian agrees to provide Roots & Wings Village with one month's advance written notice in order to be released from obligation of remaining owed tuition payments.
Roots and Wings Village retains the right to terminate the contract without notice for the following reasons:
The child(ren)’s behavior is destructive, uncontrollable, violent, or threatening to the other children or providers at the care facility. This determination is made in the sole discretion of the provider.
A parent’s/guardian’s behavior is threatening or abusive to the other children or providers at the care facility.
Childcare fees are two weeks or more delinquent.
All terminations of this type can be made effective immediately.
ROOTS AND WINGS VILLAGE LLC WAIVER:
In order to participate in Roots and Wings Village, each participant must submit completed versions of this Assumption of Risk, Waiver of Liability, and Parental Permission Form. Participants who have not completed this form will not be permitted to participate in program activities until they are received.
AGREEMENT TO PARTICIPATE
To ensure that parents/guardians understand and accept the risks of participation in Roots and Wings Village LLC (recreational program), you must indicate your understanding and agreement by signing on the appropriate lines below.
PARENT/GUARDIAN AGREEMENT
I agree to allow my child/ward to participate in the recreational program and affirm that my child’s/ward’s participation is completely voluntary. I understand that there are risks inherent in the activities my child will engage in at the recreational program which may cause serious injury or even death. I also understand that, despite safety precautions, the recreational program cannot guarantee that my child/ward will not be injured. My child/ward and I are willing to assume these risks. To minimize the risk, I have instructed my child/ward to obey all the rules, regulations and instructions of the recreational program.
ASSUMPTION OF RISK, WAIVER OF LIABILITY, RELEASE, & AGREEMENT NOT TO SUE: In exchange for permission for me and/or my child/ward to participate in Roots and Wings Village LLC’s recreational program, I voluntarily agree the following release from liability on my own behalf and on behalf of my child/ward:
TO ASSUME FULL RESPONSIBILITY FOR ANY RISKS OR LOSS, OR PERSONAL INJURY INCLUDING DEATH that may be sustained by me/my child/ward, or any loss or damage to property owned by me/my child/ward, as a result of participating in, or traveling to or from the recreational program.
I, on my own behalf, and also as a parent and/or guardian on behalf of the minor child/ward identified below, release, waive, discharge, hold harmless, and agree not to sue Roots and Wings Village LLC, Danielle Zola Maxwell, Kerin Santos, and Shanti Mandir, its officers, directors, employees, volunteers, agents, landlords, lessees, sponsors and franchisees (hereafter the “Released Parties”) from any and all liability, claims, actions, demands, expenses, attorneys fees, breach of contract actions, breach of statutory duty, or other duty of care, warranty, strict liability actions, and causes of action whatsoever, that I might now have or may acquire in the future, arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child/ward, my person or other persons, and to my child’s/ward’s property, my property or other persons’ property, arising out of or in connection with, or caused in any manner by my participation or my child’s/ward’s participation in any Roots and Wings Village LLC programs.
I acknowledge I hereby have been advised to consult, and have consulted, with my physician
and/or with my child’s/ward’s physician with respect to any past or present injury, illness, health
problem or any other condition or medication that I and/or my child now have, previously have
had and/or now may have that may affect my and/or my child’s/ward’s participation and ability to
participate in and to endure the Roots and Wings Village LLC sessions, classes, groups or
workshops.
In the event that I and/or my child/ward becomes ill or injured during or as a result of participation in any Roots and Wings Village LLC programs, I hereby authorize the Released Parties to arrange for such emergency medical attention as they, in their sole judgment, may deem to be required to preserve my life and/or health of myself or my child/ward. I hereby release, discharge and hold harmless the Released Parties, as well as any person or entity that provides such emergency medical attention, from any and all liability in connection with any injury to my or my child’s/ward’s person or property arising in connection with or as a result of such emergency medical treatment.
I acknowledge that this program is not intended to be educational in nature and any educational outcomes are not guaranteed. I, the parent/guardian, agree that I am solely responsible for my child’s/ward’s education.
PHOTO RELEASE: I give permission for photographs to be taken of me/my child/ward while participating in the recreational program to be used in marketing/public relations material in the promotion of the recreational program.
